As a patient, you have the right to examine your medical records at the approval of your physician and to receive answers to your questions about the information, unless restricted by law, within a reasonable time frame. As a department of the hospital, the Spine Medicine Institute adheres to the policies and procedures regarding medical record requests established by Saint Alphonsus.

To request a copy of your medical record, download and
complete the form below and visit the health information management
office at Saint Alphonsus, located on the first floor of the
south tower past the patient elevators. If you have questions
regarding access to your medical records, please call the health
information management office at 208-367-2101.
